Hopkins Fire Department serve as our Hennepin County Hub to scale the Community Health Program! In less than 1 year we have reduced repeat 911 calls to the cities senior living facilities by 90% and the sites are thrilled to add our CH data to their QAPI measures.
This model in Hopkins has allowed us to create Action Teams for Age Friendly MN for a 10 year project with MN Blueprint on Aging in Senior Emergency Preparedness. As well as ACO’s working with DHS for proactive emergency preparedness for group homes!
As expert system changers in this space, we lead with a a simple message- “A 911 Call is a failure of prevention and we all have a responsibility to do our part when sharing community resources.”
The West Fargo & Fargo Fire Department’s Community Health Alliance serves as the states CHA HUB with West Fargo FD serving as the states CHA Training Hub for other departments. We met quarterly to discuss the next steps to connect residents with the right solution at the right time to support their immediate medical need with community stakeholder engagement! The community stakeholders invest their time, expertise and collaborate with the commitment to supporting the Fire Department in this Community Risk Reduction Model.
The Moorhead Fire Department’s Community Health Alliance is the first in the Red River Valley to have this Community Health Program partner with County Public Health and Beth's Place to create an Opioid Response Team to offer on scene connections to treatment and support. This pilot is set to go "live" November 2024. The ORT Hub for this model will eventually scale to meet the needs of those who need it most. The Team will meet monthly to discuss the next steps to connect residents with the right solution at the right time to support their immediate medical need with community stakeholder engagement!
Duluth Fire Department's Community Health Program will serve as the BIG First Class City in our Program with a Mayo Ambulance Service, Multiple Hospital Systems and Senior Livings Sites all working in a Community Risk Reduction Model. A Community Health Mobile Integrated Health Program will be created in collaboration with these Health Large Health Systems working together to reduce the overall burden on the Fire and EMS and have strategic priorities that will serve as a model to scale in the Arrowhead Region.
Dayton Fire Department is set in a pocket where city meets rural. With a paid on call model, the City of Dayton is committed to connecting their residents to the resources they need. With over 20% of this community on county services, the lack of resources within the city are broad and the access to these needed resources are at the forefront as we partner with Hennepin County Public Health and North Memorial to bridge the gaps and connect the right resources at the right time. This CHA will serve as a model to expand the Community Risk Reduction efforts in other rural areas.
Albert Lea Fire Department is not new to community health! Our role is to create a Community Health Alliance within this Mayo service area and bring the community, stakeholders and health/service providers together to meet the needs of all Albert Lea residents. We are piloting our Health Tech - Fall Prevention Technology with partner Zemplee from Silicon Valley, CA and Twin Cities Physicians to bridge the gaps in the senior living buildings for lift assist and fall 911 calls. This CHA will serve as a model for the Southern MN Region.

The goal of this project is to collaboratively address health inequities and connect and educate partnering agencies with community leaders from migrant communities, immigrant communities, aging populations, the LGBTQIA+ community, and various diverse religious and race identifiers. Creating a Health Equity Data Analysis focused on barriers to accessing primary care, social determinants of health for this geographic area and the factors of health equity therein, for any and all populations who face health inequities. Extend the use of Community Health Workers to embed with numerous fire and public safety agencies within our Community Health Model to address health equity in targeted populations.
St Louis Park High School and Care Resource Connection created a Healthcare Exploration for Nursing Assistant students to work with the SLP CHA to build real world connections and networks for the High Schools BIPOC and first generation students to have equity in access to a career pathway. The first year was to lay the groundwork for bringing in stakeholders to reinvest back into the community by mentoring the students.
The St Louis Park CNA Program was made possible by connecting the Spring Lake Park Nursing Assistant Class to help build the program and sharing resources. We thank Bill and Jenny Neiss for all their help!

The second year allowed the students to have the opportunity post Covid to complete their practicum hours with their SLP CHA Mentors in the nursing homes and assisted livings. The students voices and feedback allowed for a CNA Toolkit for students to continue their healthcare career exploration and turn it into a career pathway.
We were so excited to kick off our Community Listening Forums this week at Banfill Crossings in Fridley! We had great conversations about the needs of our seniors and aging in place.
We were so excited to be awarded this grant to scale our Community Health Program across the State of Minnesota. Stay tuned as we continue to help find the right staffing model to bridge the gaps!
We were so excited to write this grant and have the St Louis Park Fire Department have a fulltime care coordinator.
